Register for a Library Card

Please complete the entire form. Upon completion you will receive a DO_NOT_REPLY email. Within 24 – 48 hours your temporary barcode number and new temporary pin will again be sent to this email. Please read the email carefully. You will be given full access to the Carnegie-Stout Public Library databases and E content for 30 days only. The barcode and pin will no longer be valid after 30 days.
